Teen Driver Challenge Forms

Once you have received confirmation that you are registered for the Teen Driver Challenge course, download and fill out the appropriate form below. Choose the form where you will be taking the course. This paperwork is REQUIRED and must be submitted at least 2 WEEKS PRIOR TO CLASS.  Please complete all forms and attach copies of your teen’s driver’s license/permit, vehicle insurance card and registration.
